What insurance usually covers
Medically necessary detox, inpatient and residential treatment, partial hospitalization, intensive outpatient and outpatient care are commonly covered, subject to your deductible, coinsurance and network. Under the Affordable Care Act, Marketplace plans can't set yearly or lifetime dollar limits on substance use treatment and can't deny coverage for a pre-existing condition.
Your parity rights
The Mental Health Parity and Addiction Equity Act requires plans covering substance use benefits to apply copays, deductibles, day limits and rules like prior authorization no more restrictively than for medical and surgical care. Plans must share their medical necessity criteria on request and explain any denial.
